  2. Fill out the Membership Form
  3. After you fill out this form, drop it and a $25.00 check (made out to Stone Creek Pickleball Club) in a bank bag in the bin marked “Payments and Forms” and you will be officially enrolled. If a couple is enrolling, please list both full names, email and phone number and make the check for $50.00.
